| SENSITIZATION PROGRAMMES AND TRAININGS |

| TRAINING OBJECTIVES |
| |
1. To enable doctors and other Health Care Professionals to integrate HIV testing and counseling as components of :- a) Routine clinical practice b) Antenatal care, so as to prevent the transmission of HIV to infants.
2. To enable physicians and other Health Care Professionals to maintain and improve safety of the work place from blood-borne and other infections.
3. To assist the physicians and other Health Care Professionals to comply with standardized care guidelines for Persons Living with HIV/AIDS (PLHAs) based on NACO and CDC, GAP India guidelines.
4. To be able to improve clinical competence of the physicians.
5. To help the entire medical fraternity to understand the myths and misconceptions associated with the disease. This in turn will enable them to overcome the fear of stigma and discrimination associated with the disease.
In order to accomplish the above, APAIDSCON evolved a TOT (Training of Trainers) strategy, where master trainers were identified from each partnering institution of the consortium to train each and every Health Care Professional within the institute. For this, each institution identified 2 physicians as Master Trainers. Initially it was envisaged that the Master Trainers should be from the Department of Internal Medicine. But, due to their limited numbers and busy schedule, the partnering institutions nominated physicians from departments of Community Medicine and Microbiology also. Considering the diverse make-up of the group, it was felt that there was a need to conduct a sensitization program and Training Needs Assessment Workshop for them.
1. Sensitization and Training Needs Assessment Workshop
Three days Sensitization and Training Needs Assessment Workshop for the Master Trainers was conducted from the 4th to the 6th Jan 2006, at Mediciti Institute of Medical Sciences, Ghanpur. A total of 24 participants from 11 Partnering Institutes participated in the training programme. The training programme was inaugurated by Mrs. Damayanthi, Project Director AIDS Control Society. As group based brain storming session followed by discussion helped in developing a training program based on the needs of the Institution.
2. Training Of Trainers for Master Trainers
A two day formalized training was organized for the Master Trainers on 3rd and 4th April 2006. During this the Master Trainers were provided with a lot of information in the form of PowerPoint presentations and text. Besides lectures the training comprised of group discussions, case studies and interactive sessions. The Master Trainers were sensitized about their role and the way in which they have to conduct trainings in their institutes. The budget for Institution level training prepared by APAIDSCON was discussed and allocated. The number of Master Trainers trained to date is 30. After the Master Trainers training program held in April 2006 the Partnering Institutions have started sensitizing doctors/faculty within their Institute. Institutional Level Trainings At the Institutional level a total of 1116 doctors have been sensitized through 38 sensitization programmes.
3. Clinical Training Program
An Intense Hands-on Clinical Training program was organized by CDC and APAIDSCON at Rural Development Trust Hospital, Ananthpur from 4th to 9th September 2006. The trainees were given the unique opportunity of examining HIV patients with clinical bedside case discussions. The program was designed to offer a stimulating opportunity for learning, questioning, interacting and practicing HIV medicine. The trainees were also given the opportunity to the psychosocial problems faced by the HIV positive patients. They were provided with literature, PEP case presentations, VCDs, HIV protocols and guidelines in treating adult and pediatric cases. 15 doctors underwent this training.
4. Training for Obstetricians and Gynecologists
For the enhancement of comprehensive HIV/AIDS service delivery by Medical Officers APSACS organized a 3 days training program from 4th January 2007 to 6th January 2007 at A.P. Chest Hospital for the Medical Officers. CDC provided supervisory and technical support with CHAI, LEPRA and APAIDSCON as key partners. The group comprised of doctors from Primary Health Centers of some of the districts and doctors from Partnering Institutions of APAIDSCON. Besides classroom sessions, case discussions and bedside hands-on-experience was given to them. 17 doctors attended the program.
5. Needs Assessment and Sensitization Programme for Nurses
Nursing staff form the backbone of any hospital service and in fact the services provided by them are considered to assess the quality of care provided at the hospital. They are the people who are in close touch with the patients and the compassion and care shown by them plays an important role in the recovery of the patient.. In the case of HIV-AIDS, the nurses have an important role to play, it is important that they are sensitized and oriented to different aspects of care and support required by the unfortunate victims of the dreaded disease. Hence, APAIDSCON gives lot of importance to the training of the nursing staff.
OBJECTIVES :- The objectives of the current program were:-
1. PRIMARY OBJECTIVES: -
a. Enhance the overall knowledge of nurses on HIV/AIDS
b. Specifically talk about, understand and discuss nurse related issues pertaining to
diagnosis, management and care.
2. SECONDARY OBJECTIVES:- The focus of the training will be on Preventive Aspects
a. Infection Control
b. Universal Precautions
On 12th and 13th April 2207 a needs assessment and sensitization programme was held for nurses of the Partnering Institutions of APAIDSCON. It was a one and half day programme. In order to identify the learning requirements of the participant nurses and to develop a better understanding of the existing capacities of the nurses, a needs assessment exercise was conducted. This was a group brainstorming exercise. Following this, on the second half of day 1 i.e. 12th April 2007, the core topics were taken up in the form of PowerPoint presentations, group discussions and interactive sessions.
The participants were provided with CDs containing copies of presentations and modules. Hard copy of the reference and facilitator module was also provided to them.
Pre and post test was conducted and feedback was also obtained from them in the form of questionnaires.

Dr. Dora Warren, Country Director, CDC visited Mediciti on 15.02.2007. Dr. G S Saluja, Project Director, made a power point presentation on the activities of APAIDSCON to her.

A meeting of the Steering Committee of APAIDSCON was held on 23.02.2007 at Hotel Sai Prakash, Hyderabad. The meeting was chaired by Dr. G S Saluja, Project Director, APAIDSCON. The meeting was also attended by the CDC representatives form AP and Chennai.
Dr. Saluja welcomed the members and introduced the following three new members of the APAIDSCON Team who joined during the months of January and February 2007:
a. Mr. B V Soma Sastry, Manager Operations
b. Mr. P S Murty, Manager Communications
c. Dr. Bindu Asok Kumar, PPP Consultant
He explained in detail, through a power point presentation, the PEPFAR targets and indicated the participating institution-wise targets to be achieved during the year 2. The focus of the discussion was on the utilization of VCTC, PPTCT and provision for care and support for the HIV positive patients.
Mr. Asok Kumar, Project Director, APSACS later addressed the members of the Committee. He emphasized the need to treat HIV positives in all the hospitals of partnering institutes as it would also be useful for the students graduating as medical students. The PD also wanted the Consortium to concentrate on and improve the STI services as studies reveal that almost 90% of the positive cases are transmitted through STI/RTI.
The PD informed the members about the ambitious campaign of 0/7 in AP. The aim of the campaign, he pointed out to track all ANC mothers, and ensure that no child is born HIV positive. The consumables etc for the project are being provided by the APSACS and the new born child will be tracked until the age of 24 months.
In response to the complaints of delay in supplying equipment for ICTC, Mr. Ashok Kumar clarified that the Partnering Institutes can procure equipments and consumables for ICTC from the open market after following the procedures and ensuring that the purchases fall within the allotted budget.
He also urged the Steering Committee members to develop working contacts with the District Project Management Units (DPMUs) and Additional DMHO for any information related to HIV. The DPMUs have been created in 10 high prevalence districts to assist the ADMHO in their efforts to eradicate HIV.
Lastly he was very critical on the irregular reporting of the colleges and informed the members that from April 2007 the laboratory technicians and counselors posted at the VCTC/PPTCT centers of the medical colleges will be under the direct supervision of APAIDSCON instead of LEPRA.
Dr. Michael Friedman, Associate Director, South India, CDC informed the members about the release of the year 2 budget to SHARE India. He stressed on the importance having regular core committee meetings at all the partnering institutes. He also emphasized that each member Institution should take up the responsibility of delivering and treating HIV cases. He emphasized the need to work as a team instead of as individual entities.
Dr. Michael also sought suggestions from the members on the HIV Fellowship Program to be initiated by APAIDSCON in collaboration with the APSACS and supported by the CDC. The Fellowship Program is on the lines of the program being conducted by GHTM Tambaram. It already has the approval of Project Director, APSACS. Some members expressed interest in the program and sought details.
Dr. John Schneider, Project Director, PHMI introduced to the members the concept of conducting HIV Elective (optional) as any changes in the curriculum. May be time consuming and requires University approvals. He sought suggestions from the members on how to start and subsequently improve upon the concept.

CDC biannual meet was held at Mount Opera. 4 of the APAIDSCON staff attended the meet. All the grantees made a power point presentation of the activities in the past six months. Dr. Vijayaraghavan, Director Research, made the presentation on behalf of APAIDSCON.

CDC team consisting of Ms. Jeanine Ambrosio, Deputy Director of Operations, Delhi and Mr. Gopalachary Ramesh, Administrative Manger, Chennai made a one day visit to Mediciti to review the Standard Operating Procedures and Management (SOP) systems to assess functionality of systems and compliance with CDC recommendations and federal guidelines/standards. The copy pf the report has been received and the recommendations are being implemented

Integrated Counseling and Testing Centers (ICTC)
Through the initiation and follow up of APAIDSCON, an Integrated VCTC-PPTCT center, now called Integrated Counseling and Testing Center ( ICTC ) for the partnering institutes was approved by APSACS. For each sanctioned ICTC, APSACS released a sum of Rs 1.20 lakhs to APAIDSCON for purchasing various start up equipment, disposables etc.
CARE AND SUPPORT SERVICES
The need for provision of anti-retroviral drugs forms an integral part of care& support of people living with HIV/AIDS ( PLHAS ),hence Mediciti Institute of Medical Sciences,Ghanpur, the leading partnering institute of APAIDSCON has launched a pilot program where PLHAs will receive subsidized antiretroviral drugs. To enhance the service delivery, the following partnering medical colleges have been identified to provide care and support services.
1. Mediciti Institute of Medical Sciences.
2. GSL Medical College, Rajamundry
3. NRI Academy of Sciences, Guntur.
4. Mamata Medical College, Khammam
5. Kamineni Institute of Medical Sciences, Narketpally
